Welcome to the CO-CREATE-Ex Study

This study is a collaboration between UC San Diego, San Ysidro Health, the Global Action Research Center, and Duke Clinical Research Institute. It is funded by the National Institutes of Health.

The goal of this study is to learn how to improve access to COVID-19 testing in the communities served by San Ysidro Health (SYH). If you sign up for the study, you will be asked to fill out a questionnaire, and will be offered a free COVID-19 test. You will also be offered a $20 gift card for completing the questionnaire after your first visit.

Participation in this study is voluntary, and you can cancel your participation at any time. Your healthcare at SYH will not change whether or not you decide to participate in this study. Participation in this study will benefit you because you will receive a free COVID-19 test.

The most common risk of this study is discomfort when swabbing your nose for the COVID-19 test. Another risk is the potential loss of confidentiality.
